Streamline Your First Spine Appointment

Many patients have already had studies such as MRIs, CT scans, bone scans or X-rays performed prior to being referred to our office. These may have been ordered by a chiropractor, family physician or other referring physician. Our doctors rely on the actual images from these studies, rather than a written report alone. Therefore, gathering images regarding your neck or back from as many prior studies as possible and bringing them to your initial appointment will help your physician more efficiently and accurately discuss your diagnosis and treatment options with you. In our office, we have the capability to review images in either printed film formats or electronically on CDs (compact discs).
 
Before your first appointment, visit our Patient Information  page to see how fees are collected, which insurance carriers are accepted and read our Notice of Privacy Practices. You many also print our Patient Information form, complete it at home, and bring it with you to your first appointment.
 
Patients being seen by Dr. Van Pelt or Dr. Lucas also can download a patient history form, fill it out at their convenience and bring it with them to their appointment along with the Patient Information form.
